Download and complete the Company Data Information Form (PDF) only if requested to do so by a Springs Utilities staff person. Return the completed form either by e-mail or fax as instructed by the requestor. Download the latest version (PDF form) of the Request for Taxpayer Identification Number and Certification (W-9) from the IRS Web site. Because this form requires a signature, it must be either faxed or m ailed to the requestor. Both of the above forms are required in order to do business with Springs Utilities. Failure to provide either or both of these forms, as requested, will cause delays or failed transactions with your company. Please note that any completed Company Data Information and/or Request for Taxpayer Identification forms received without a request by a Springs Utilities staff person will be discarded. The Optional Vendor ACH Enrollment Form (PDF) is made available on this Web page to those vendors/suppliers who wish to receive their invoice payments from Springs Utilities electronically.
